   　なお、直接当社にご来社いただいてのご請求はお受けいたしかねますので、あらかじめご了承ください。

　個人情報及び特定個人情報の開示等のご請求を行う場合は、個人情報等開示等請求書【PDF：164KB】をダウンロードし、印刷していただき、必要事項をご記入し、記名押印のうえ、以下に掲げる本人確認のための書類を同封し、4.（2）の開示等のご請求先あてに、郵便でご請求ください。
　請求書類は当社所定のものに限るものとし、それ以外の書式では一切お受けいたしません。
　また、個人情報及び特定個人情報の開示等のご請求に際してご提出いただいた書面等は、ご返却いたしませんので、あらかじめご了承ください。

• 本人確認（代理人の本人確認を含む。）のための書類
  　請求を行う際には、本人であることをご確認させていただくため、本人確認用の書類（行政官庁、またはそれに相当すると当社が判断できる機関が発行した証明書の写し）をご提出いただきます。
  　写真の表示のある証明書（例：旅券（パスポート）、運転免許証等、個人番号カード等）は1種類、その他の確認書類（例：健康保険証、年金手帳、印鑑登録証明書、住民票の写し等）は2種類の写しを添付していただきます。
• 法定代理人の資格を証明する書類
  　代理人からの請求の際には、本人確認書類とあわせて、代理人の代理権とその身元を確認させていただくため、次に定めるa及びb両方の確認用書類をご提出いただきます。
  a　代理人確認書類　本請求書を作成・提出いただいた方が代理人欄に記載された代理人と同一であることを確認させていただくために必要な書類です。
  　写真の表示のある証明書（例：旅券（パスポート）、運転免許証等、個人番号カード等）は1種類、その他の確認書類（例：健康保険証、年金手帳、印鑑登録証明書、住民票の写し等）は2種類の写しを添付していただきます。
  b　（代理人の）代理権確認書類
  　本請求書に記載された請求事項に関して、代理人が代理権を有していることを確認させていただくために必要な書類です。
  　法定代理人の場合は法定代理権を確認できる書類（例：親権者の場合は戸籍謄本、成年後見人の場合は成年後見登記事項証明書等）の写しを、法定代理人以外の代理人は代理権を確認できる書類（委任状、個人情報等の開示請求等に関する代理権の約定がある契約書等）の写しを、1部添付していただきます。

8. How to notify the result of your request for disclosure of personal information and specific personal information

We will notify the name and address of the claimant who filled out the request form for disclosure of personal information, etc., by mail (postal limited mail or similar method) in writing. In addition, we will notify you by adding the reason for the case where we do not disclose it.
